Transferring your medical records is an important step when changing clinics, and we want to make the process is as seamless as possible for you and your family. Whether you are bringing your records to Double Bay Doctors or sending them to a new clinic, understanding the steps involved can help ensure everything runs smoothly.
Below is a simple guide on how to transfer your records to us or from us to another clinic.
TRANSFERRING TO DOUBLE BAY DOCTORS
If you're looking to bring your records from your previous clinic to us, here's how to do it:
Complete our online form so that we can gather all of the necessary details to process your request.
The time it takes for us to receive your records depends on your previous clinic and how they send your file. Some clinics also charge an admin fee for transfer of medical records and will contact you for payment prior to releasing them to us.
TRANSFERRING FROM DOUBLE BAY DOCTORS
If you are looking to transfer your medical records from Double Bay Doctors to an external healthcare provider, you will firstly need to complete the below online form so that we can gather all of the necessary information to process your request.
Once we have received your online request, it will be forwarded on to your GP for authorisation.
Once your GP has authorised the release of your records, we will send them electronically to your new clinic free of charge in XML format. Please note that if your new clinic does not accept medical records in this format, there may be an admin fee of $35 for us to collate and post your records.
Please allow up to 10 business days for your request to be completed.
