Care Plans
A GP Chronic Condition Management Plan (GPCCMP) is a written plan developed by your GP (or other eligible medical practitioner) to help manage long-term health conditions. If you are eligible, a GPCCMP provides access to Medicare-subsidised allied health services such as physiotherapy, podiatry, exercise physiology, dietetics and more - replacing what was previously referred to as an Enhanced Primary Care Plan (EPC) or GP Management Plan (GPMP).

WHO IS ELIGIBLE?
To be eligible for a GPCCMP, you must have a chronic or terminal medical condition that requires ongoing management. Chronic conditions are generally those present for six months or longer and may include asthma, diabetes, cardiovascular disease, cancer, musculoskeletal conditions, stroke and many others.
Eligibility is based on your clinical needs. Your GP will assess whether a GPCCMP is appropriate for you, and whether you would also benefit from Medicare-subsidised allied health services as part of a Team Care Arrangement (TCA).
HOW DO I GET ONE?
To obtain a GPCCMP, you will need to book a standard 15-minute appointment with your usual GP.
Please be aware that preparing a GPCCMP requires the full appointment time and cannot be completed during a consultation where other issues are being managed. If you need to discuss additional concerns, you will need to book a long (30-minute) appointment or arrange a separate standard appointment.
For continuity and quality of care, it’s important that you see your regular GP for the preparation of your GPCCMP and for any required reviews.
