
Enhanced Primary Care Plans
An Enhanced Primary Care Plan (EPC) is a referral that is written by your GP which provides eligible patients with Medicare-subsidised allied health appointments, like physio and podiatry. An EPC may also be referred to as a GP Management Plan (GPMP).
WHO IS ELIGIBLE?
To be eligible for an EPC or GPMP, a patient must have a chronic or terminal medical condition with or without complex care needs.
A chronic health condition is defined as one that has been present for 6 months or more. It includes, but is not limited to, asthma, cancer, cardiovascular illness, diabetes mellitus, musculoskeletal conditions and stroke.
Your GP will need to assess your eligibility for an EPC plan.
